About New York’s Social Care Network (SCN) Program
Social Care Networks (SCNs) are part of New York State Medicaid. They help eligible Medicaid members connect with services that address health-related social needs. Through Foodie, eligible participants can receive boxes with ready-to-eat food delivered to their door.
If approved, you will receive a box of ready-to-eat food delivered to your door every week. Most applicants we serve qualify, but final eligibility is based on program requirements.
SCNs were established through New York’s Medicaid 1115 Waiver initiative. Eligible Medicaid members can be screened for social needs and connected with qualifying services. Food support provided through this program may be available at no cost to eligible members.
